CarePayment (we, us, our) values your privacy and takes steps to protect your information in accordance with applicable law. Please see below for our commitment related to the information you provide to us as a visitor to or user of our websites, portals or mobile applications that link to this Statement (each a “Site”). Also, please see steps that you can take to protect your identity and security.  If you have a CarePayment account originated by Republic Bank & Trust Company, your activities associated with the account are covered by Republic Bank & Trust’s Privacy Notice, available here. Our Privacy Statement continues to apply to your provision of information to us over the sites.

1. Agreement to this Privacy Statement and Changes.By accessing, visiting and/or using this Site, you, the visitor, acknowledge that you have read, understand and agree to be bound by this Privacy Statement. If you do not agree to be bound by this Statement, do not use this Site. CarePayment reserves the right to change this Statement at any time by posting an updated version to this Site. You will be bound by those changes if you visit this Site after those changes have been posted. The date this Statement was last modified is identified at the end of this page. Please check this Statement periodically for changes.

2. Information CarePayment Collects. CarePayment collects the following information from and about visitors to this Site:

a. Information That You Provide. CarePayment and our service providers receive information that your provide when you access this Site, including, for example, your name, address, phone number, email address, Social Security number if you open an account, and other similar information. That information is collected when you create an account or apply for credit on this Site, when you complete surveys or forms on this Site, and when you report a problem with this Site. We also keep the correspondence you send us.

b. Transaction Data. We collect information about your payments and other transactions through this Site.

c. Visits; IP Addresses. Our servers track information about the number of visitors to this Site and their viewing behavior. We collect technical information about your visits to this Site including, if available, the date and time you access this Site, your unique IP address, the Internet address of the website from which you linked directly to this Site, your operating system, browser type, and other device information, the pages you view and how long you view them, other traffic data, and your browser and plug-in configurations. We use this information to administer the Site.

d. Cookies and Similar Technologies. This Site, like most other commercial websites, uses "cookies" and web beacons. Cookies are bits of electronic information that a website operator can transfer to your hard drive to help tailor, and keep records of, your visit to that website. Web beacons are small pieces of code installed on webpages that allow the collection of certain user information like IP address and pages visited.  Cookies allow us to recognize your computer when you return to this Site. While they can identify your zip code, they do not identify you by name. Cookies store personal preference and user data and track user trends and patterns. That information allows us to customize your visits to your individual preferences and to speed up your searches. Cookies also allow us to better estimate the usage patterns of visitors to this Site. This gives you the chance to decide whether or not to accept it. While you may refuse to accept cookies, doing so may prevent you from accessing or using certain features of this Site. Unless you have changed your browser setting to prevent cookies, our system will issue cookies when you log on this Site.  If the Site you are using is a mobile app, we will not use cookies but we may collect advertising identifiers as well as the other information described in this section 3.

e. Geolocation:  When using one of our mobile apps, we may also collect your geolocation for analytics to improve the service and for advertising based on your location.

f. We also receive information from your healthcare provider in order to open and maintain your CarePayment account or other billing services.

g. In addition, we may combine the information we collect with information we obtain from third party data sources.

3. Third Party Cookies and Third Data Collection.  We may work with various third parties such as advertisers, advertising networks, advertising servers, and analytics companies that use various technologies and methods, including cookies, to collect data about your use of the Sites and your online activity generally.  These third party online companies may automatically collect information about your online activities over time and across different websites and apps when you visit the Sites. See section 9 below for more information.

4. CarePayment's Use of Your Information. CarePayment uses information about you in order to provide the services described in this Site or that we otherwise provide to you, to improve the Site's content, organization, layout and other features, to communicate with you about our products and other offerings, and to provide required notices.

a. Email Communications with You. From time to time, CarePayment may use your personal information to send email messages to you regarding your account, to respond to any emails or other communications you send to us, and/or to advise you of any problems with the Site. In addition, if you are a registered user of this Site, we may use a bulk email service to send messages with information about services and events that we believe you may be interested in. 5. Disclosure of Your Information. We may disclose your information to our subsidiaries and other affiliated companies, including CarePayment. We may also disclose your information to unrelated persons in the following circumstances:

a. Investigations. We may disclose your information to unrelated persons in the investigation of any actual or suspected: (a) breach of this Statement or our Terms of Use, (b) illegal activity, (c) fraud, (d) intellectual property infringement, or (d) threats of violence or other harm.

b. To Provide Services to You. We may disclose your information to your health care provider, to the financial institution we partner with to provide you services, or other persons as necessary to provide any services that you have requested.

c. Third Parties who provide services to us. We may disclose your information as necessary for our third party service providers to provide services to us. For example, we may engage other companies to host, operate and maintain this Site, to analyze data, provide marketing assistance, process your credit card payments, process your credit application, perform inventories, audits or assessments, and provide other services. In those situations, we will provide them access only to the personal information they need in order to do their jobs and only if they have agreed in writing not to use your information for any other purpose, and not to disclose your information to anyone outside of their organizations other than as required by law and to maintain industry class information safeguards.

d. Merger and Acquisition Activity. We may disclose your information to prospective investors in, or buyers of, CarePayment or its subsidiaries or affiliates in connection their due diligence and/or ultimate investment or acquisition of our business or assets (and in the case of an acquisition, your information will be one of the assets transferred).

e. Other Cases. We may disclose your information if we in good-faith believe that doing so is either required of us or is reasonably necessary (a) to comply with applicable law, including court orders, subpoenas, statutes and regulations; (b) to protect and defend CarePayment's or a third party’s rights or property; and/or (c) in an emergency to protect the personal safety of CarePayment's customers, visitors or the public.

f. With Your Permission. We may disclose your information to unrelated persons when you have consented to our doing so.

g. Deidentified Information. We may share de-identified information about your activities with third parties.

6. CarePayment's Use of Your Information. CarePayment uses information about you in order to provide the services described in this Site or that we otherwise provide to you, to improve the Site's content, organization, layout and other features, to communicate with you about our products and other offerings, and to provide required notices.

7. How CarePayment Stores Your Information. When we receive your information, we will use security processes and procedures consistent with industry standards that are designed to prevent unauthorized access to, or use of, that information. For example, we store information that we collect from this Site on secure servers in physically secure facilities. We restrict access to nonpublic personal information about you within CarePayment to those who need to know that information to provide products or services to you. We also maintain physical, electronic/technological, and procedural safeguards that comply with federal regulations to guard your nonpublic personal information. If you are a patient, former patient or guarantor of a patient obligation to a Provider, or other payee, and if you use a credit card to make payments on an account, that information will be provided to the bank that CarePayment has engaged to process those payments (the "Credit Card Processor) through a direct link, and you will be subject to the privacy policy and terms of use of the Credit Card Processor. CarePayment will not store your credit card information. We encrypt all credit card information using SSL technology.

8. Links to Other Websites. When you visit this Site, you may have the opportunity to access or link to third party websites or online services, including social media services.  Those websites and online services are beyond our control, and those other websites may not have privacy disclosures and security protections, or their privacy practices or security protections may not be as protective of your personal information as ours. These other websites may send their own cookies to visitors, collect data or solicit personal information. We recommend that you review and understand the privacy practices of the websites you visit, whether you visit those websites directly or through a link from a page on this Site.

9. Third Party Online Company Use of Information.  The third party advertising entities described in section 3 above use cookies, web beacons, advertising identifiers and similar technologies to track how the Sites are used, where users go and what they do after they leave the Sites, to your devices and to serve more relevant ads either on the Sites or on third party websites and online services that you visit.   We also contract with third party analytics companies who use cookies to provide us with aggregate data about site traffic.  This Privacy Statement does not cover the practices of these third party companies.  While we work with them to improve our Sites and your experience, we do not control their technologies.

10. Do Not Track. 11. Log-on Information. If we issue, or you select, a user name, password, personal identification number or other log-on information in connection with your use of this Site, a credit application, a payment transaction, or any other interaction with this Site, you are responsible for keeping that log-on information confidential. Do not share your log-on information with anyone else.

12. Notice. If you have any reason to suspect that your log-on information or any of your other personal information has been compromised, please notify us immediately. 13. Policy Regarding Children. This Site is intended for use by adults, aged 18 and over. CarePayment does not intentionally collect any information about children or minors.

14. The Sites are for use by people who live in the United States. This Site is intended for U.S. users only. Accordingly, if you are visiting this Site from outside of the United States, the information collected through the Site will be processed in and subject to the laws of the United States, which may not provide the same level protection for your personal information as your home country.

16. Changes. CarePayment may change this Statement at any time by posting an updated version to this Site. Steps to Protect Your Identity and Security

Your identity protection and security is a top priority.  Let us know right away if you receive suspicious communications or if anything does not seem right with your CarePayment account by calling our toll free number, 1-866-625-8532.  Thieves sometimes use legitimate company brand names and logos to trick consumers into divulging their personal information in order to commit identity theft and to takeover financial accounts.

Tips:

• If you receive a suspicious call, do not provide the caller with any information, end the call, and contact CarePayment right away.  When you call CarePayment, we will ask some basic authentication questions to ensure and protect your identity.

• If you receive a suspicious email, do not click any links and contact CarePayment right away.  Use your email filters to reduce spam and undesirable emails.

• If you receive a suspicious text, do not reply or click on any links and contact CarePayment right away.

• Please keep in mind that for electronic payments to your CarePayment account, we will not transact with your credit card or checking/savings account without your preauthorized agreement.  This may have been provided by mail, phone or online.  For reoccurring automated payments, CarePayment also provides advance notice on monthly billing statements.  If there were ever an error with processing a payment, CarePayment will contact you if we perceive doing so is necessary.  Just be sure to verify the caller, check the caller ID, and if anything seems suspicious, hang up and contact our regular customer service line at 1-866-625-8532.  Read your statements and report anything suspicious or unknown to your bank or to CarePayment as relevant.

• When connecting to a website, double check that it is secure before proving any information.  This can be done while connecting with your browser by checking the website address for HTTPS, the ‘S’ means it’s secure and a padlock symbol to the right of the website address.  An unsecured website would only indicate HTTP with no padlock.  Any variation in this approach could be a suspicious website attempting to trick you into divulging personal information.

• Be very careful when commenting and posting content onto social media and never provide personally identifiable information.

• Be sure to protect your devices by using complex passwords, swipe screens and fingerprint scans.  This means your computer, tablet of mobile phone.  So if your device is ever lost or stolen, it will be harder for crooks to access your accounts and information.  Always use up-to-date antivirus software and avoid transacting and sending sensitive information while connected to an unfamiliar or suspicious WiFi or other public hotspot.  Only download apps and files from reputable sources, like the Play Store or the App Store.  Be sure to regularly install operating system and firmware updates.

• If you become a victim of identity theft or just want additional tips and resources, the FTC has an excellent website.  Also be sure to periodically request and review a free copy of your credit report from annualcreditreport.com and don’t be fooled by gimmicks and impersonating websites.
